Below you will find two forms. First, indicate what you would like engraved on your brick. The second page will ask you to provide your contact information. Once you have completed these two forms your order will be finished. We anticipate completion of this project by mid-August so all orders must be finalized by Monday, May 27, 2019. No exceptions or late orders can be accepted due to our timeline for completion.
